Effects of alcohol on risk factors for cardiovascular disease

A summary paper on the effects of alcohol consumption on biologic mechanisms associated with coronary heart disease provides an excellent review of a large number of intervention studies in humans. Appropriate analyses were done and the results are presented in a very clear fashion, although there was little discussion of the separate, independent effects of alcohol and polyphenols on risk factors.

The trials the authors reviewed have demonstrated that the moderate intake of alcoholic beverages leads to increases in HDL-cholesterol (good cholesterol), apolipoprotein A1, and adiponectin and decreases in fibrinogen, all factors associated with a lower risk of heart disease. The findings described in this paper strengthen the case for a causal link between alcohol intake and a reduced risk of coronary heart disease, suggesting that the lower risk of heart disease observed among moderate drinkers is caused by the alcoholic beverage itself, and not by other associated lifestyle factors.

The reviewers independently selected studies that examined adults without known cardiovascular disease and that compared fasting levels of specific biological markers associated with coronary heart disease after alcohol use with those after a period of no alcohol use (controls). A total of 4,690 articles were screened for eligibility, the full texts of 124 studies reviewed, and 63 relevant articles selected. Of 63 eligible studies, 44 on 13 biomarkers were meta-analysed in fixed or random effects models. Quality was assessed by sensitivity analysis of studies grouped by design. Analyses were stratified by type of beverage (wine, beer, spirits).

The authors concluded that favourable changes in several cardiovascular biomarkers (higher levels of high density lipoprotein cholesterol and adiponectin and lower levels of fibrinogen) provide indirect pathophysiological support for a protective effect of moderate alcohol use on coronary heart disease.

Ibuprofen may reduce risk of developing Parkinson's disease

A new study by Harvard School of Public Health (HSPH) researchers shows that adults who regularly take ibuprofen, a non-steroidal anti-inflammatory drug (NSAID), have about one-third less risk of developing Parkinson's disease than non-users.

"There is no cure for Parkinson's disease, so the possibility that ibuprofen, an existing and relatively non-toxic drug, could help protect against the disease is captivating," said senior author Alberto Ascherio, professor of epidemiology and nutrition at HSPH.

The study will be published online March 2, 2011, in Neurology and is scheduled to appear in the March 8, 2011, print issue.

Parkinson's disease, a progressive nervous disease occurring generally after age 50, affects at least half a million Americans, according to the National Institute of Neurological Disorders and Stroke. About 50,000 new cases are reported each year, with the number expected to increase as the U.S. population ages. It is hypothesized that ibuprofen may reduce inflammation in the brain that may contribute to the disease.

Prior studies showed a reduced Parkinson's disease risk among NSAIDS users, but most did not differentiate between ibuprofen and other non-aspirin NSAIDs.

In the new study, Ascherio, lead author Xiang Gao, research scientist at HSPH and associate epidemiologist in the Channing Laboratory at Brigham and Women's Hospital, and colleagues analyzed data from nearly 99,000 women enrolled in the Brigham and Women's Hospital-based Nurses' Health Study and over 37,000 men in the Health Professionals Follow-Up Study. The researchers identified 291 cases (156 men and 135 women) of Parkinson's disease during their six-year follow-up study (1998-2004 in women; 2000-2006 in men). Based on questionnaires, the researchers analyzed the patients' use of ibuprofen (e.g. Advil, Motrin, Nuprin), aspirin or aspirin-containing products, other anti-inflammatory pain relievers (e.g., Aleve, Naprosyn), and acetaminophen (e.g., Tylenol). (Although not an NSAID, acetaminophen was included because it's similarly used to treat pain.) Age, smoking, diet, caffeine, and other variables also were considered.

"We observed that men and women who used ibuprofen two or more times per week were about 38% less likely to develop Parkinson's disease than those who regularly used aspirin, acetaminophen, or other NSAIDs," Gao said. "Our findings suggest that ibuprofen could be a potential neuroprotective agent against Parkinson's disease, however, the exact mechanism is unknown."

These findings raise hope that a readily available, inexpensive drug could help to treat Parkinson's disease. "Because the loss of brain cells that leads to Parkinson's disease occurs over a decade or more, a possible explanation of our findings is that use of ibuprofen protects these cells. If so, use of ibuprofen could help slow the disease's progression," Gao said.

The findings do not mean that people who already have Parkinson's disease should begin taking ibuprofen, Ascherio added. "Although generally perceived as safe, ibuprofen can have side effects, such as increased risk of gastrointestinal bleeding. Whether this risk is compensated by a slowing of the disease progression should be investigated under rigorous supervision in a randomized clinical trial," he said.

Alcohol consumption helps stave off dementia

Experts agree that long-term alcohol abuse is detrimental to memory function and can cause neuro-degenerative disease. However, according to a study published in Age and Ageing by Oxford University Press today, there is evidence that light-to-moderate alcohol consumption may decrease the risk of cognitive decline or dementia.

Estimates from various studies have suggested the prevalence of alcohol-related dementia to be about 10% of all cases of dementia. Now researchers have found after analyzing 23 longitudinal studies of subjects aged 65 years and older that the impact of small amounts of alcohol was associated with lower incidence rates of overall dementia and Alzheimer dementia, but not of vascular dementia and cognitive decline. It is still an open question whether different alcoholic beverages, such as beer, wine, and spirits, all have a similar effect. Some studies have shown a positive effect of wine only, which may be due either to the level of ethanol, the complex mixture that comprises wine, or to the healthier life-style ascribed to wine drinkers.

A total of 3,327 patients were interviewed in their homes by trained investigators (physicians, psychologists, gerontologists) and reassessed one and a half years and three years later. Information on the cognitive status of those who had died in the interim was collected from family members, caregivers or primary care physicians.

Among the 3,327 patients interviewed at baseline, 84.8% (n=2,820) could be personally interviewed one and a half years later and 73.9% (n=2,460) three years later. For the vast majority of subjects who could not be personally interviewed, systematic assessments (follow-up 1: 482; follow-up 2: 336) focusing particularly on dementia could be obtained from GPs, relatives or caregivers. Within three years, follow-up assessments were unavailable for only 49 subjects (1.5%). Proxy information could be obtained for 98.0% (n=295) of the 301 patients who had died in the interim. Since dementia is associated with a higher mortality rate, proxy information is particularly important in order to avoid underestimation of incident dementia cases.

At baseline there were 3,202 persons without dementia. Alcohol consumption information was available for 3,180 subjects:

50.0% were abstinent
24.8% consumed less than one drink (10 grams of alcohol) per day
12.8% consumed 10-19 grams of alcohol per day
12.4% consumed 20 or more grams per day
A small subgroup of 25 participants fulfilled the criteria of harmful drinking (>60 grams of alcohol per day for men, respectively >40 grams for women)
One man (>120 grams of alcohol per day) and one woman (>80 grams of alcohol per day) reported an extremely high consumption of alcohol
Among the consumers of alcohol almost half (48.6%) drank wine only
29.0% drank beer only
22.4% drank mixed alcohol beverages (wine, beer, or spirits)
Alcohol consumption was significantly associated with male gender, younger age, higher level of education, not living alone, and not being depressed.

The calculation of incident cases of dementia is based on 3,202 subjects who had no dementia at baseline. Within the follow-up period of three years:

217 cases of dementia (6.8%) were diagnosed, whereby 111 subjects (3.5%) suffered from Alzheimer dementia. Due to the relatively small numbers, other subgroups of dementia (vascular dementia: n=42; other specific dementia, e.g. dementia in Parkinson's disease, Lewy body dementia, alcohol dementia: n=14; dementia with unknown aetiology: n=50) were not considered in the following analyses.
Univariate and multivariate analyses revealed that alcohol consumption was significantly associated with a lower incidence of overall dementia and Alzheimer dementia. In line with a large-scale study also based on GP attenders aged 75 years and older, the study found that light-to-moderate alcohol consumption was associated with relatively good physical and mental health. This three-year follow-up study included, at baseline, only those subjects 75 years of age and older, the mean age was 80.2 years, much higher than that in most other studies.

Turmeric might be "the right spice" to fight colon cancer

Turmeric, a bright yellow spice from south Asia belonging to the ginger family, is the main ingredient in curries — and ancient wisdom suggests that it's also good for your health. Taking this wisdom to the laboratory, Tel Aviv University researchers have discovered that turmeric's active ingredient called curcumin amplifies the therapeutic activity of highly toxic anti-inflammatory drugs used to fight colon cancer when used at high doses.

Dr. Shahar Lev-Ari of Tel Aviv University's School of Public Health at the Sackler Faculty of Medicine and his colleagues have found that curcumin can fight cancer when used in combination with a popular anti-inflammatory drug, alleviating the inflammatory response caused when cancer takes root in the body. A treatment based on this finding has already had promising results in human clinical trials.

"Although more testing will be needed before a possible new drug treatment is developed," says Dr. Lev-Ari, "one could combine curcumin with a lower dose of a cancer anti-inflammatory drug, to better fight colon cancer." The results of the new study have been published in the journal Therapeutic Advances in Gastroenterology.

Alleviating unwanted side effects

Research in the last few decades has shown that cancer is linked to inflammation. Several lines of evidence demonstrate that chronic inflammation in the stomach can cause gastric cancer and that inflammation in the liver from hepatitis can lead to liver cancer.

Dr. Lev-Ari and his colleagues found that Celecoxib, a popular anti-inflammatory drug commonly used to treat arthritis, also inhibits proliferation of colon cancer in laboratory settings. Curcumin increases the anti-cancer and anti-inflammatory effects of Celecoxib while reducing its dose, thus reducing its toxic side-effects, including the rate of heart attack and stroke.

The effect of using a curcumin concentrate to improve the effects of cancer drugs was first proposed by Dr. Lev-Ari when he was a graduate student at Tel Aviv University's Sackler Faculty of Medicine under the supervision of Prof. Nadir Arber and Prof. Dov Lichtenberg.

Both co-supervisors were eager to test the possible health benefits described in folk medicine but were looking for hard evidence. "We would like to use this treatment for patients with all types of cancers," says Prof. Arber. "It has the promise of being an important life-extending therapy, particularly for non-curable pancreatic cancer, suggested by the very promising results we achieved for 20 pancreatic cancer patients."

Putting shelved cancer drugs back into circulation


Previous in vitro and in vivo experiments conducted by the Tel Aviv University team show that curcumin inhibits an enzyme known as COX-2 (cyclooxygenase-2), believed to cause inflammation. The team’s research demonstrates that curcumin neutralizes oxygen free radicals, which are believed to play an important role in carcinogenesis.

These effects may be the basis for drug treatment of both inflammation and cancer through the combination of curcumin and Celecoxib. And it may also help return previously shelved potent anticancer drugs — taken out of use due to high toxicity — back to the market under lower dosage indications.

Apple antioxidant could extend average lifespan

Scientists are reporting the first evidence that consumption of a healthful antioxidant substance in apples extends the average lifespan of test animals, and does so by 10 percent. The new results, obtained with fruit flies — stand-ins for humans in hundreds of research projects each year — bolster similar findings on apple antioxidants in other animal tests. The study,
"Apple Polyphenols Extend the Mean Lifespan of Drosophila melanogaster", appears in ACS's Journal of Agricultural and Food Chemistry.

Zhen-Yu Chen and colleagues note that damaging substances generated in the body, termed free radicals, cause undesirable changes believed to be involved in the aging process and some diseases. Substances known as antioxidants can combat this damage. Fruits and vegetables in the diet, especially brightly colored foods like tomatoes, broccoli, blueberries, and apples are excellent sources of antioxidants. A previous study with other test animals hinted that an In the current report, the researchers studied whether different apple antioxidants, known as polyphenols, could do the same thing in fruit flies.

The researchers found that apple polyphenols not only prolonged the average lifespan of fruit flies but helped preserve their ability to walk, climb and move about. In addition, apple polyphenols reversed the levels of various biochemical substances found in older fruit flies and used as markers for age-related deterioration and approaching death. Chen and colleagues note that the results support those from other studies, including one in which women who often ate apples had a 13-22 percent decrease in the risk of heart disease, and polish the apple's popular culture image as a healthy food.

In my last report, I mentioned that while I'm not worried about osteoporosis (other than spelling, again) I might be happy taking biosphophonates to fight it - since they seemed to add 5 years to life expectancies - now we have some specific evidence:

The use of bisphosphonates for more than one year was associated with a 50 percent reduction in the risk of postmenopausal colorectal cancer.


But because the bisphosphonates act much like statins, I may be already covered.

Health benefits of eating tomatoes

Eating more tomatoes and tomato products can make people healthier and decrease the risk of conditions such as cancer, osteoporosis and cardiovascular disease, according to a review article the American Journal of Lifestyle Medicine, (published by SAGE).

Of all the non-starchy vegetables, Americans eat more tomatoes and tomato products than any others. Researchers Britt Burton-Freeman, PhD, MS, and Kristin Reimers, PhD, RD of the National Center for Food Safety & Technology, Illinois Institute of Technology and ConAgra Foods, Inc., looked at the current research to discover the role tomato products play in health and disease risk reduction.

The researchers found that tomatoes are the biggest source of dietary lycopene; a powerful antioxidant that, unlike nutrients in most fresh fruits and vegetables, has even greater bioavailability after cooking and processing. Tomatoes also contain other protective mechanisms, such as antithrombotic and anti-inflammatory functions. Research has additionally found a relationship between eating tomatoes and a lower risk of certain cancers as well as other conditions, including cardiovascular disease, osteoporosis, ultraviolet light–induced skin damage, and cognitive dysfunction.

Tomatoes are widely available, people of all ages and cultures like them, they are cost-effective, and are available in many forms. "Leveraging emerging science about tomatoes and tomato products may be one simple and effective strategy to help individuals increase vegetable intake, leading to improved overall eating patterns, and ultimately, better health." write the authors.

"Tomatoes are the most important non-starchy vegetable in the American diet. Research underscores the relationship between consuming tomatoes and reduced risk of cancer, heart disease, and other conditions," the authors conclude. "The evidence also suggests that consumption of tomatoes should be recommended because of the nutritional benefits and because it may be a simple and effective strategy for increasing overall vegetable intake."

The article is particularly timely since the recently released Dietary Guidelines for Americans 2010 moved tomatoes to a newly established category of "orange/red" fruits and vegetables to encourage higher consumption of these healthy foods.
