Exercise increases the body’s own ‘cannabis’ which reduces chronic inflammation

 

In a new study, published in Gut Microbes, experts from the University of Nottingham found that exercise intervention in people with arthritis, did not just reduce their pain, but it also lowered the levels of inflammatory substances (called cytokines). It also increased levels of cannabis-like substances produced by their own bodies, called endocannabinoids. Interestingly, the way exercise resulted in these changes was by altering the gut microbes.

Exercise is known to decrease chronic inflammation, which in turn causes many diseases including cancer, arthritis and heart disease, but little is known as to how it reduces inflammation.

A group of scientists, led by Professor Ana Valdes from the School of Medicine at the University, tested 78 people with arthritis. Thirty-eight of them carried out 15 minutes of muscle strengthening exercises every day for six weeks, and 40 did nothing.

At the end of the study, participants who did the exercise intervention had not only reduced their pain, but they also had more microbes in their guts of the kind that produce anti-inflammatory substances, lower levels of cytokines and higher levels of endocannabinoids.

The increase in endocannabinoids was strongly linked to changes in the gut microbes and anti-inflammatory substances produced by gut microbes called SCFAS.  In fact, at least one third of the anti-inflammatory effects of the gut microbiome was due to the increase in endocannabinoids.

Doctor Amrita Vijay, a Research Fellow in the School of Medicine and first author of the paper, said: “Our study clearly shows that exercise increases the body’s own cannabis-type substances. Which can have a positive impact on many conditions.

“As interest in cannabidiol oil and other supplements increases, it is important to know that simple lifestyle interventions like exercise can modulate endocannabinoids.”

Drinking caffeinated coffee: both beneficial and harmful short-term health effects

 Drinking caffeinated coffee appears to have both beneficial and harmful short-term health effects: increased abnormal heartbeats, increased physical activity and reduced sleep duration, according to late-breaking research presented today at the American Heart Association’s Scientific Sessions 2021. “Coffee is the most commonly consumed beverage in the world, yet its health effects remain uncertain,” said study author Gregory Marcus, M.D., M.A.S., associate chief of cardiology for research and endowed professor of atrial fibrillation research at the University of California, San Francisco. “While the majority of long-term observational studies have suggested multiple potential benefits of drinking coffee, this is the first randomized trial to investigate the real-time, physiologic consequences of coffee consumption.”

Marcus and colleagues enrolled 100 adult volunteers, and they were assigned to wear continuously recording ECG devices (to track heart rhythm), wrist-worn devices to track physical activity and sleep; and continuous glucose monitors to track blood sugar levels for two weeks. The participants were an average age of 38 years, 51% were women and 48% were white. Researchers also obtained DNA saliva samples from the participants to assess genetic variants that may affect caffeine metabolism.

Participants were then randomly assigned to either avoid or consume coffee for no more than two consecutive days each for 14 consecutive days. Coffee and espresso consumption were recorded in real time via a “time stamp button” on the ECG monitor, and researchers tracked trips to coffee shops with geotracking. In addition, participants completed daily questionnaires to detail how much coffee they had consumed every morning.

The analysis found that coffee consumption was associated with a 54% increase in premature ventricular contractions, a type of abnormal heartbeat originating in the lower heart chambers reported to feel like a skipped heartbeat. In contrast, drinking more coffee was associated with fewer episodes of supraventricular tachycardia, an abnormally rapid heart rhythm arising from the upper heart chambers.

Consuming coffee was consistently associated with more physical activity as well as less sleep. Specifically:

  • Participants who consumed coffee logged more than 1,000 additional steps per day compared to days when they did not drink coffee.
  • On the days participants drank coffee, they had 36 fewer minutes of sleep per night according to their Fitbit devices.
  • Drinking more than one coffee drink more than doubled the number of irregular heartbeats arising from the heart’s lower chambers.
  • Each additional cup of coffee consumed was associated with nearly 600 more steps per day and 18 fewer minutes of sleep per night.
  • There were no differences in continuously recorded glucose measured when the study participants consumed versus avoided coffee.

These findings were corroborated by analyses of adherence to their randomization assignment and amplified when more versus less coffee was consumed.

“More physical activity, which appears to be prompted by coffee consumption, has numerous health benefits, such as reduced risks of Type 2 diabetes and several cancers, and is associated with greater longevity,” Marcus said. “On the other hand, reduced sleep is associated with a variety of adverse psychiatric, neurologic and cardiovascular outcomes. More frequent abnormal heartbeats from the upper heart chambers influence risk of atrial fibrillation, and more frequent abnormal beats from the lower chambers, or ventricles, increase the risk of heart failure. These results highlight the complex relationship between coffee and health.”

Long-term, low-dose aspirin did not affect risk of dementia in adults with type 2 diabetes

While daily low-dose aspirin may be prescribed to reduce the risk of having a second heart attack or clot-related stroke, it is also associated with internal bleeding including brain bleeds. “The overall effect of aspirin on dementia and cognitive impairment was uncertain,” said study author, Jane Armitage, F.R.C.P., a professor of clinical trials and epidemiology at the Nuffield Department of Population Health at the University of Oxford in the U.K. “Aspirin may be protective for dementia by preventing some strokes due to blockages, or it may increase the risk because of bleeding into the brain.”

According to the American College of Cardiology/American Heart Association 2019 primary prevention recommendations, adults without known cardiovascular disease should only take aspirin for heart attack and stroke prevention if they have the highest risk of cardiovascular disease and a very low risk of bleeding. Recent draft guidance from the U.S. Preventive Services Task Force agrees with this approach – daily low-dose aspirin should not be prescribed for cardiovascular disease prevention in adults who do not have existing cardiovascular disease. However, adults with previously diagnosed cardiovascular disease or a previous cardiac event such as a heart attack or stroke, should continue to take daily low-dose aspirin if prescribed by their physician.

Researchers aimed to assess the effects of low-dose aspirin on the risk of dementia and cognitive impairment in participants enrolled in the ASCEND (A Study of Cardiovascular Events in Diabetes) trial. The ASCEND trial included more than 15,000 adults with type 2 diabetes living in the U.K. who had not experienced a stroke, heart attack or other circulatory issue, and who did not have dementia at the beginning of the study. Half of the participants took a single 100-milligram aspirin, and half received an identical placebo pill daily. The study tracked participants for nearly nine years, with an average of about seven years of treatment and almost two additional years of follow up.

At the end of follow-up, presence of dementia was determined by several methods: results of a cognitive function test (Telephone Interview of Cognitive Status and verbal fluency or the Healthy Minds test); diagnoses listed in hospital admission data or death records; and other indicators of cognitive impairment listed in electronic health records. Researchers also noted the occurrence of serious illness, heart attacks, strokes or major internal bleeding.

Researchers found 1,146 participants experienced “broad dementia,” meaning dementia, cognitive impairment or delirium or confusion; and they were prescribed dementia medications or received a referral to a memory clinic or geriatric psychiatry.

“The results show no clear effect of daily low-dose aspirin on the risk of dementia, with a non-significant 9% proportional reduction in risk. However, the uncertainty around this 9% benefit ranged from a 19% reduction in dementia risk to a 2% increase. This is reassuring that an increase in the risk of dementia is unlikely for the millions of people worldwide who regularly take aspirin to protect against the risk of heart attack and stroke,” said Armitage. “The results mean a modest benefit of daily low-dose aspirin on risk of dementia is possible, however, we need studies with more people developing dementia to be sure.”

The researchers did find, however, that serious vascular events, such as a heart attack or major bleeding episodes like a stroke, were associated with dementia. There were 990 participants who survived a major vascular event and 496 who survived a major bleed during the study.

Specifically:

  • Study participants who experienced a major vascular event were almost two and a half times more likely to experience dementia, memory loss, confusion or mental decline during the study compared to the participants who did not have a major vascular event.
  • Those who had a major bleed were twice as likely to experience dementia, memory loss, confusion or mental decline than those who did not have a bleed.

A limitation of the study is there may not be enough cases of reported dementia to make a clear assessment of the effect of daily low-dose aspirin on the risk of dementia. “A larger study with more cases of dementia may be able to detect any benefits or harms. We plan to continue to follow the trial participants for several more years to see if more cases of dementia emerge,” Armitage said.

Low-frequency intermittent fasting prompts anti-inflammatory response

 Intermittent fasting may not only be a hot dieting trend, but it also has broader health benefits, including helping to fight inflammation, according to a new study from researchers at the Intermountain Healthcare Heart Institute in Salt Lake City.

Previous research has shown that intermittent fasting, an eating pattern that cycles between periods of fasting and eating, may improve health markers not related to weight. Now, the new Intermountain research shows that intermittent fasting raises the levels of galectin-3, a protein tied to inflammatory response.

“Inflammation is associated with higher risk of developing multiple chronic diseases, including diabetes and heart disease. We’re encouraged to see evidence that intermittent fasting is prompting the body to fight inflammation and lowering those risks,” said Benjamin Horne, PhD, principal investigator of the study and director of cardiovascular and genetic epidemiology at the Intermountain Healthcare Heart Institute.

Findings of the study will be presented on Saturday, November, 13, at the American Heart Association’s Scientific Sessions 2021, which are being held virtually this year.

These results are part of Intermountain’s WONDERFUL Trial studying intermittent fasting, which found that intermittent fasting causes declines in the metabolic syndrome score (MSS) and insulin resistance.  

This specific study examined 67 patients aged 21 to 70 who all had at least one metabolic syndrome feature or type 2 diabetes. Participants also weren’t taking anti-diabetic or statin medication and had elevated LDL cholesterol levels.

Of the 67 patients studied, 36 were prescribed an intermittent fasting schedule: twice a week water-only 24-hour fasting for four weeks, then once a week water-only 24 hour-fasting for 22 weeks. Fasts could not be done on consecutive days. The remaining 31 participants made no changes to their diet or lifestyle.

After 26 weeks, researchers then measured participants’ galectin-3, and found that it was higher in the intermittent fasting group. They also found lower rates of HOMA-IR (insulin resistance) and MSS (metabolic syndrome), which researchers believe may be similar to the reported effects of SGLT-2 inhibitors, a class of drugs used to lower high glucose levels in type 2 diabetes patients.

“In finding higher levels of galectin-3 in patients who fasted, these results provide an interesting mechanism potentially involved in helping reduce the risk of heart failure and diabetes,” said Dr. Horne, who added that a few members of the trial team completed the same regime before the study started to make sure that it was doable and not overly taxing to participants.

“Unlike some IF diet plans that are incredibly restrictive and promise magic weight loss, this isn’t a drastic form of fasting. The best routine is one that patients can stick to over the long term, and this study shows that even occasional fasting can have positive health effects,” he added.

Consuming a low amount of caffeine during pregnancy could help to reduce gestational diabetes risk

 Good news for pregnant coffee lovers: Consuming a low amount of caffeine during pregnancy could help to reduce gestational diabetes risk, according to researchers in the Perelman School of Medicine at the University of Pennsylvania and the National Institutes of Health. The American College of Obstetricians and Gynecologists (ACOG) recommends that pregnant women limit their caffeine consumption to less than 200 mg (about two, six-ounce cups) per day. The recommendations are based on studies that suggest potential associations with pregnancy loss and fetal growth at higher caffeine levels. However, there remains limited data on the link between caffeine and maternal health outcomes.

To better understand this association, researchers studied prospective data from 2,529 pregnant participants who were enrolled in the National Institute of Child Health and Human Development (NICHD) Fetal Growth Studies-Singleton Cohort at 12 U.S. clinical centers between 2009 and 2013.

At enrollment and at each office visit thereafter, women reported their weekly intake of caffeinated coffee, caffeinated tea, soda, and energy drinks. Concentrations of caffeine were also measured in the participants’ plasma at 10 to 13 weeks into their pregnancies. The researchers then matched their caffeine consumption with primary outcomes: clinical diagnoses of gestational diabetes, gestational hypertension, and preeclampsia.

The research team found that caffeinated beverage intake at 10 to 13 weeks gestation was not related to gestational diabetes risk. During the second trimester, drinking up to 100 mg of caffeine per day was associated with a 47 percent reduction in diabetes risk. There were no statistically significant differences in blood pressure, preeclampsia, or hypertension between those who did and did not drink caffeine during pregnancy.

The researchers note that the findings are consistent with studies that have found that caffeine has been associated with improved energy balance and decreased fat mass. They also say that they cannot rule out that these findings are due to other constituents of coffee and tea such as phytochemicals, which may impact inflammation and insulin resistance, leading to a lower risk for gestational diabetes.

However, past studies from the same group have shown that caffeine consumption during pregnancy, even in amounts less than the recommended 200 mg per day, was associated with smaller neonatal anthropometric measurements, according to Hinkle.

“It would not be advised for women who are non-drinkers to initiate caffeinated beverage consumption for the purpose of lowering gestational diabetes risk,” she said. “But our findings may provide some reassurance to women who already are consuming low to moderate levels of caffeine that such consumption likely will not increase their maternal health risks.”


Adverse effects of obesity medications: Further research into orlistat, lorcaserin needed

 An essential part of obesity care is making sure treatments are safe for patients.

Abdulrahman Alsuhibani, a doctoral student in the University of Cincinnati's James L. Winkle College of Pharmacy, and his colleagues in the UC division of health outcomes recently published a study in the International Journal of Clinical Pharmacy analyzing the adverse events attributed to the use of anti-obesity medications. 

The research team reviewed data from the Food and Drug Administration’s Adverse Event Reporting System database from 2013-20. Patients, physicians and other health care workers submit any suspected side effect from an anti-obesity medication to the system.

In the seven years analyzed, more than 18,000 unique adverse events were reported from more than 15,000 patients taking various anti-obesity medications. The study reported that 4.9% of the reported adverse events were deaths of patients, with 7.6% listed as life-threatening events, 35% requiring hospitalization and 5.9% described as disability cases.


Alsuhibani noted that while the overall fatality rate of 4.9% is considered high, this study does not show causality but rather informs where additional research and monitoring may be needed.

“It is necessary to continue and systematically monitor the safety of anti-obesity medications to optimize patient anti-obesity therapy,” he said. “Observational studies do not show causality; however, it shows the hypothesis of using real-world data to try to show some connections and association.” 


Alsuhibani said a unique finding of the study is that deaths represented 4% of all adverse events attributed to orlistat, an over-the-counter anti-obesity medication. Since orlistat is the only anti-obesity drug available to the public without a prescription, he said there needs to be further investigations of the fatal cases potentially associated with using this medication.


The most common adverse events reported among users of anti-obesity medications were nausea, vomiting, dizziness and headache. There were also 1,765 reports of cardiovascular disease, 1,327 reports of kidney complications and 194 reports of cancer. 

The study began in light of the fact that several anti-obesity medications have been pulled from the market due to documented adverse events, including the withdrawal in February 2020 of the drug lorcaserin following reports that it may cause cancer. 

Regarding lorcaserin, Alsuhibani said the study that led to its removal from the market had been questioned in the field due to many concerns, with the researchers not fully disclosing the information around the lorcaserin/cancers potential risk association. Alsuhibani’s study found that 16% of all reported adverse events attributed to lorcaserin use were reports of cancer.
